Service Appointments Now Required at North/South Annex Locations

Please read the following in it’s entirety. Failure to adhere to the scheduling policies or submit the correct forms, applications and fees may result in the inability to render service.  


For your safety and convenience, appointments are now required for all services at our North Annex and South Service Centers:                                                                                                                                         To help us prepare for your visit and better serve you, please indicate (in the "Notes" box on the scheduling page) the type of service to be rendered and the number of applications being submitted.
We value everyone’s schedule, so please arrive at your appointment on time. Applicants more than five minutes late will be asked to schedule a new appointment.


FOR PASSPORTS: We encourage you to visit our website Passport Application Services or contact the scheduling location to determine the appropriate forms, fees and payment types specific to your application(s).  Failure to provide correct forms, fees and payment types may result in the rejection of your application(s) and require a new scheduling date.
All passport applications require two separate payments: Passport Application fee (payable by check or money order) and a standard processing fee (payable by check, money order or credit/debit card - excluding Amex).                                                                                                                                                                                                                                                                                                                                           *Note: Our office does not accept renewal passport applications for adults. *Applicants applying for adult passport renewals must renew by mail. FOR NOTARY APPLICATIONS: We encourage you to visit our website Notary Commission/Renewal Services or contact the scheduling location to determine the appropriate forms, fees and payment types specific to your application(s).  Failure to provide correct forms, fees and payment types may result in the rejection of your application(s) and require a new scheduling date.

**Note: Applicants renewing their notary commission qualify for mail-in renewal services. If you are interested in renewing your notary by mail, please visit Mail-In Notary Renewal Option.

FOR REAL ESTATE FILINGS: All documents must be in their original form (no copies). Filing fees vary. Please see our fee schedule to determine the appropriate fee for your filing. Failure to provide the correct fees and payment types may result in the rejection of your filing(s) and require a new scheduling date.

FOR COPIES (OTHER MISCELLANEOUS SERVICES): Please indicate the case number and/or defendants name. Additionally, please contact the location prior to your visit to ensure that your copies are available. Please see our fee schedule to determine the appropriate fee(s) for your request. Failure to provide the appropriate information, fee(s) and payment types may result in the rejection of your request(s) and require a new scheduling date.
